- Description: Across 19 randomized trials (3,502 patients), adding metformin to chemotherapy did not improve progression-free or overall survival (HR 1.08 both), but raised response rates (OR 1.59) without extra severe toxicity.

Key Takeaway

Adding metformin to chemotherapy did not help patients live longer or stay progression-free in a meta-analysis (a pooled analysis) of 19 randomized trials with 3,502 people. Tumors did shrink a bit more often, with no extra severe side effects. Survival hints in subgroups are exploratory — not proven.

Researchers pooled 19 randomized controlled trials — the gold standard, where patients are randomly assigned to a treatment or control group — testing the diabetes drug metformin alongside chemotherapy. On the primary endpoints (the outcomes the analysis was built around): progression-free survival, the time without the cancer growing, was unchanged (hazard ratio HR 1.08 — HR 1 means no difference), and overall survival was unchanged too (HR 1.08).

The one positive: overall response rate — the share of patients whose tumors shrink — improved (odds ratio OR 1.59, 95% CI 1.02–2.46; CI is the uncertainty range), driven by complete responses, meaning tumors disappearing on scans. Exploratory subgroup looks (secondary slicing, not the main test) hint at a survival benefit at 1,500 mg/day with platinum chemo (HR 0.61) — hypothesis-generating only. Severe grade ≥3 side effects (the most serious category) were no more common with metformin (OR 1.10).

Bottom line: metformin is safe to combine but is no survival booster overall; the subgroup signal needs its own prospective trials. ## In plain words

**What happened:** Researchers pooled 19 randomized trials (3,502 people) that added the diabetes drug metformin to standard chemotherapy.

**Why it matters:** Metformin is cheap and widely tolerated, and lab studies hint it could boost chemo — this analysis asked whether that holds in real randomized trials.

**How strong is the evidence:** A strong design, and the honest answer is mixed: no survival benefit overall; tumors shrank somewhat more often; survival signals in subgroups are exploratory.

**What it means for you:** If you are on chemo, this changes nothing — do not start metformin on your own; it must fit your treatment plan. Discuss any add-on with your oncology team.
